Travel Rate Effective January 1, 2010 .50 Forms Below Updated January 5, 2010 with .50 rate CCBOE Travel Form - Blank This is the official CCOBE travel form to be used for requesting travel payment. The form is in Microsoft Word format and does not include any automatic calculation field. It can be used for printing out and making copies to fill in by hand. CCBOE Travel Form - Sum This is the official CCOBE travel form to be used for requesting travel payment. The form is in Microsoft Word format. It includes an automatic calculation field that can help you total your travel if it is only one page in length and you complete your form on a computer. Fill in the row values with your mileage and meals. Then right mouse click on the total area at the bottom and choose 'update field'. You will need to repeat this procedure for all four total fields at the bottom of the form.
