Technology purchases should come through the Technology Office on a Board purchase order and be aligned with the technology plan. Include the name and room number of the individual on the purchase order. Also include the Technology Plan Goal and Benchmark number (Ex: Objective 1-3).

This is the official CCOBE travel form to be used for requesting travel payment. The form is in Microsoft Word format.
It includes an automatic calculation field that can help you total your travel if it is only one page in length and you complete your form on a computer. Fill in the row values with your mileage and meals. Then right mouse click on the total area at the bottom and choose 'update field'. You will need to repeat this procedure for all four total fields at the bottom of the form.
